Notice, Map, and Boundary Description
To locate your October 2006 address, first look at the class boundary map, then go to Google Maps using the link below, type in the address, and then zoom out so you can see Highway 64, Highway 1, and Highway 55 (Williams St). If your address falls within these three highways (except for the triangle at the intersection of Highway 64 and Highway 1), you are likely in the Recommended Evacuation Zone.
THE FINAL SUBMISSION DATE FOR NEW CLAIMS (OCTOBER 20, 2009) IS NOW PAST, NO NEW CLAIMS WILL BE ACCEPTED.
A NOTICE OF DEFICIENCY HAS BEEN SENT TO CLASS MEMBERS WHOSE CLAIM FORMS ARE INCOMPLETE FOR THE FOLLOWING REASONS.
MISSING COPY OF ID: A copy of picture ID for the head of household (the person competing the claim form) is required for each claim submitted:
CLAIM FORM NOT SIGNED: All claim forms must be signed by the head of household who completed the claim form.
MISSING PROOF OF EVACUATION FOR SECONDARY ZONE ONLY: All claims submitted for the Secondary Evacuation Zone must include proof of evacuation. This can be a hotel bill or a notarized affidavit from some one you stayed with.
DID NOT ANSWER QUESTIONS CONCERNING EVACUATION AND/OR PRIOR PAYMENTS: All claim forms must include answers to the evacuation and prior payment sections.
MISSING PROOF OF LOSS FOR BUSINESS CLAIMS ONLY: All Business claims submitted must include documents that clearly provide proof of loss. The proof of loss should clearly show the loss your business incurred on those days the business was closed due to the fire. The proof of loss can be copies of tax returns, daily or weekly deposits for prior weeks or months, business overhead and payments to employees who could not work, or other accounting from a qualified CPA or accountant.
